tiviLux V15.2 — Provider Session Fix

FIXED
- Resolves "Provider account not found" when opening Live TV / guide after V15/V15.1.
- All ES modules now share one canonical API/session module instance.
- Removes internal version-query mismatches that split session.account between modules.
- Player, guide, library, search, favorites, and sports now use the same selected provider state.
- Adds stale provider-id cleanup when session data refreshes.
- Bumps PWA/service-worker cache so the fixed JavaScript replaces V15.1 cleanly.

PRESERVED
- V15 web/mobile performance overhaul.
- V15.1 real NFL / MLB / PPV channel counts.
- Startup/update screen and provider database.
